A nonprofit organization on Long Island wants to add a Licensed Practical Nurse (LPN) at one of their residential facilities in Suffolk County. In this role, the Licensed Practical Nurse (LPN) will be responsible for supporting the Day & Residential programs.
About the Opportunity:
- Schedule: Monday to Friday (No weekends or holidays)
- Hours: 7:30am to 4pm or 8:30am to 5pm (very flexible)
- Patient Population: Adult ID/DD OPWDD
- Patient Volume: Caseload of 16
- Setting: Residential facility
Responsibilities:
- Participate in written nursing plans of care for caseload with RN Supervisor
- Participate in planning and implementation of training of DDI personnel
- Maintain medical record for caseload
- Ensure all medications are administered and stored. as necessary
- Ensure medical follow through on all medical appointments, including any new orders and follow appointments needed
- Act as a liaison with the Pharmacy team in ensuring medications are provided to residence
- Ensure compliance with OPWDD regulatory standards and regulations
Qualifications:
- Graduate of an accredited School of Nursing
- NYS Licensed Practical Nurse (LPN)
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- Experience working with the DD / ID populations
- Knowledge of OPWDD regulations
