Summary:
The RN UAS Assessment Nurse is a hybrid (home and field-based position) for the purpose of conducting UAS initial and reassessment visits. The RN UAS Assessment Nurse will be responsible for the completion of both in-home assessments and remote assessments, tasking tools, and all other required paperwork or electronically submitted documentation related to the completion of UAS assessments. Reports to UAS Regional Manager.

Responsibilities:
- Schedule and conduct face-to-face and phone clinical assessments for potential and current Medicaid members via UAS assessments according to state guidelines, policies, procedures, and protocols.
- Perform assessments in the home setting and by telephone
- Create service plans and determine eligibility and continuing program services via the UAS.
- Conducts comprehensive assessment of all aspects of potential new members condition - clinical, social, and environmental to determine the level of services needed to appropriately keep member safe in the community.
- Gathers clinical information from various sources to address all conditions, and follows appropriate guideline, policy, procedure and clinical judgment in completing assessment requirements.
- Conducts all assessment functions in compliance with regulatory and company policies and procedures.
Requirements:
- Active NYS Registered Nurse license
- 2 years of experience working with patients in a home setting, or 1 year of UAS assessment experience
