Job Title: Director of Case Management
Job Type: Full-Time, Permanent

Responsibilities:
- Lead and inspire a multidisciplinary Case Management and Social Work team to deliver excellent, coordinated care
- Partner with hospital leadership, physicians, and staff to ensure patients receive the right care, at the right time, in the right setting
- Oversee clinical, operational, and financial performance of case management functions
- Champion best practices in utilization review, care transitions, and regulatory compliance
- Monitor metrics and drive continuous quality improvement within the department
- Serve as a strategic voice for patient advocacy, discharge planning, and resource management
Qualifications:
- Bachelor's degree in Nursing, Social Work, or related field (required); Master’s degree (preferred)
- 5–7 years of clinical experience in Case Management or Utilization Review (required)
- Minimum 3 years of leadership experience in Case Management (acute care preferred)
- Active RN or LCSW license in California (required)
- Basic Life Support (BLS) certification (required within 90 days of hire)
- CCM or ACM certification (preferred)
