VHS is looking for a qualified Nurse Practitioner - Family Medicine.

• Description: Start Date: July 21st or Eariler Duration: 90-day assignment confirmed by clientLocation: Home visits across Ocean County 6 providers needed. Please specify if your provider is comfortable starting his/her day in Ocean County.Scheduling:Must be available between 8 AM 8 PM Scheduled for an 8-hour shift within this window e.g. 8 AM 5 PM or 12 PM 8 PMOne-hour lunch break includedSchedules are released 2 3 weeks in advanceFull-time 40 hours/week Monday through Saturday flexible within that windowJob Details:Conduct 5 6 home health assessments per dayPaid for a full day regardless of patient volumePerform annual risk assessments and review of chronic conditionsNo treatment or prescribing assessment-only roleEscalate urgent findings to the patient s PCPIf applicable conduct A1C tests only other screenings left for the patientTravel required mileage reimbursed dailyEquipment: iPadBlood pressure cuffLab kits for patient useCredentialing:1 2 week turnaroundMust have an active NJ license and NJ Medicare numberQualifications:Must be Board-Certified FNP or ACNPMust be licensed in NJMileage will be reimbursed daily Will not cover travel Shift Info: Day 5x8-Hour 08:00 - 17:00
