• Board requirement (years of eligibility defined by facility): BC/BE
• Board requirement (Example EM, FM, IM): ABS
• Life Support Certifications Required: ACLS
• COVID vaccine/exemption required? Yes

LOCUM COVERAGE ASSESSMENT

• Reason for Locum need? Vacancy Maternity/Paternity
• Coverage Type: Scheduled Clinic Hours + Call
• Is the Locum Coverage needed short-term (3-6 months) or long-term (6+ months)? Short-term
• Is the Locum Coverage needed for a specific shift type: Inpatient
• Expected amount of time in Hospital: 2-4 hours per day of rounding, and remaining hours on-call with call-back obligations.
• Call Response Time: 20 mins
• Temp Privileges granted? Yes with clean files and an active license
• Start Date: January 1, 2025

• Patient Volume: 3-4 consults/10 pts. on rounds
• Support Staff: Hosp. staff only
• Will they be solo or have other surgeons on staff? Solo on call

• Bed Count: 205
• Shift Times / Coverage Details: Call - 7a-7a URGENT need for Jan through Jan 5th for call then on going needs for call and some clinic. Also need: January 13th, 15th, 20th, 21st, 23rd, 27th, 29th and 30th
• EMR: Paragon
• Trauma Center Designation: N/A
