Board Certified - REQUIRED Active WA License - REQUIRED OR IMLC (LOQ MUST BE IN HAND & UPLOADED WITH NAME CLEAR SUBMISSION) Active ACLS, BLS - REQUIRED - MUST NOTE WILLING TO OBTAIN IF PROVIDER DOES NOT HAVE Clean Malpractice - HIGHLY PREFERRED Availability M-F 8a-5p, 1-2 days a week call & 1 weekend per month - REQUIRED TO NOTE AT SUBMISSION Disclose if provider has worked at any other CHI/Dignity facilities - REQUIRED COVID VACCINATED - REQUIRED Facility Description

Outpatient Duties

See new and established hematology and oncology patients.

Required Procedures

EPIC Support Staff

A designated RN and an MA.

Reason for Coverage

Staff transitioning out and onboarding a new physician.

Patient Load

Roughly 15-20 patients per day Location
