*Responsibilities:*

* Perform in-home evaluations and develop individualized care plans
* Implement therapy interventions to improve patients' ability to perform daily living activities
* Educate patients, caregivers, and families on therapy goals and home safety
* Document visits and progress accurately and in a timely manner
* Collaborate with nurses, physicians, and other home health team members

*Qualifications:*

* Active Michigan OT license (or eligibility)
* Home Health experience preferred, but not required
* Strong communication, time management, and interpersonal skills
* Reliable transportation and ability to travel within the Hillsdale area
