Care Manager

Overview The Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family.

The Care Manager facilitates the plan of care with the objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care.

The Care Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of service, promote efficient utilization of resources and ensure a well-organized and safe plan of care is established for every patient.

Responsibilities Coordinate the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family.

Facilitate the plan of care with the objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care; identify areas for performance improvement (e.g., day-to-day workflow, education, process improvements, patient satisfaction) and drive change.

Be accountable for a designated patient caseload and plan effectively to meet patient needs, manage the length of service, promote efficient utilization of resources, and ensure a well-organized and safe plan of care for every patient.

Qualifications Education

Associate Degree in Nursing or higher is required.

Current licensure as a Registered Nurse by the NC Board of Nursing is required.

Experience

0-3 years of Medical Case Management.
