Being a Nurse Navigator LVN provides a unique opportunity to work onsite in a collaborative environment, coordinating care and guiding patients through their health journeys without direct bedside care. Be a Catalyst for Change: Use your nursing expertise to connect mobile healthcare resources, integrating services and innovations to improve patient outcomes.
Assess patient needs remotely using evidence-based protocols, helping them navigate the complex healthcare system.
Provide vital health education, support healthy behaviors, and help patients understand their care options across the healthcare continuum.
Leverage Technology: Engage in telemonitoring activities, utilizing cutting-edge tools to track progress and intervene proactively.
Lead & Collaborate: Play a key role in team care planning, mentoring colleagues, and championing new approaches in healthcare delivery.
Represent GMR’s innovative programs within the community, reaching populations that need care most but often go underserved.
Serve as a facilitator and educator for GMR’s integrated healthcare training, empowering others to deliver better care.
Deliver Excellence: Manage your time, resources, and documentation efficiently while providing outstanding customer service and support.

Valid LVN Compact State License.
Minimum of 5+ years of LVN experience in Emergency, Acute Care, or a similar fast-paced environment.
Experience with clinical data collection, ethical principles, and patient advocacy.
Graduate of an accredited nursing program.
Graduate of an accredited vocational school or nursing program preferred.
