RN/LPN Wellness Nurse

Maplewood Brewster LLC is seeking a compassionate and dedicated RN/LPN Wellness Nurse to join our Healthcare & Medical Services team in our assisted living and memory care community.

In this role, you will play a key part in supporting our resident-centered model of care, helping seniors maintain their highest possible level of independence, comfort, and wellness.

Working within a collaborative, interdisciplinary team, you will provide direct nursing care, coordinate clinical services, and build meaningful relationships with residents and their families.As an RN/LPN Wellness Nurse, you will conduct ongoing health assessments, monitor changes in condition, and implement individualized care plans tailored to each resident’s needs.

You will administer medications and treatments safely and accurately, follow established clinical protocols, and document all care and observations in accordance with professional standards and regulatory requirements.

You will also partner closely with physicians, therapists, and other healthcare providers to ensure continuity and quality of care.You will contribute significantly to our assisted living and memory care programs by supporting residents with dementia or cognitive decline, using a calm, respectful, and person-centered approach.

Your responsibilities will include monitoring behavioral and health changes, implementing non-pharmacological interventions when appropriate, and helping maintain a safe, engaging environment that supports residents’ emotional and physical well-being.

You will actively participate in care conferences, communicate updates to families, and provide education on health conditions, medications, and wellness strategies.This position is well-suited for nurses who value strong teamwork and open communication.

You will mentor and guide care associates, delegate tasks within scope, and help ensure that daily care routines reflect each resident’s preferences and care plan.
