SOUTHERN HEALTH PARTNERS INC is seeking a dedicated LPN for a 12 Hour Night Shift position within our Healthcare & Medical Services department.

Southern Health Partners Inc provides healthcare services to correctional facilities across the US, delivering quality medical, dental, and psychiatric care to incarcerated individuals while ensuring compliance with state regulations and supporting overall facility safety and health.

As a night-shift LPN, you will play a key role on the front line of care, providing compassionate and clinically sound nursing services to a unique and often underserved patient population in a secure correctional environment.In this role, you will perform comprehensive and focused patient assessments, obtain and document vital signs, and monitor changes in patient conditions throughout your shift.

You will administer medications and treatments in accordance with physician orders and state scope of practice, including oral, topical, and injectable medications, and you will provide basic wound care, dressing changes, and support for chronic disease management.

You will assist with triage for sick call and urgent needs, respond to medical emergencies under established protocols, and collaborate closely with RNs, physicians, and mental health providers to support timely, appropriate interventions.Working within a correctional facility, you will follow strict infection control, safety, and security procedures, maintaining clear communication with custody staff to promote a safe environment for patients and healthcare personnel.

You will document all care accurately and thoroughly in medical records or electronic health systems, ensuring that notes, medication administration records, and incident reports meet professional and regulatory standards.

You will provide patient education regarding medications, chronic illness, and self-care, and offer supportive, respectful interaction with individuals who may present with complex medical, mental health, and substance use histories.Southern Health Partners fosters a supportive and collaborative culture, emphasizing teamwork, compassion, and professional growth.

As an LPN on the night shift, you will participate in shift handoffs, contribute to care planning, and support continuity of care between day, evening, and night teams.
