Clinical Care Leader, Long Term Care
Job Description
Good Samaritan is seeking a compassionate and skilled Clinical Care Leader, Long Term Care to guide and coordinate high-quality clinical services for residents in our long-term care environment.

In this role, you will provide day-to-day clinical leadership for nursing and care staff, ensuring safe, evidence-based, and person-centered care across our long-term care population.
You will oversee the development, implementation, and evaluation of individualized care plans, with a focus on chronic disease management, functional support, and quality of life for older adults and medically complex residents.
Working closely with an interdisciplinary team—including physicians, advanced practice providers, therapists, social workers, and support staff—you will coordinate services, lead clinical huddles, and facilitate effective communication to support smooth transitions of care and consistent follow-through on treatment plans.
You will monitor clinical outcomes, track key quality and safety indicators, and help design and implement improvement initiatives aligned with Good Samaritan’s commitment to excellence, integrity, and community engagement.
The Clinical Care Leader will serve as a clinical resource and mentor for nursing staff, providing coaching, training, and feedback to strengthen clinical judgment, documentation, infection control practices, and regulatory compliance.
You will support survey readiness and adherence to state and federal long-term care regulations, internal policies, and best-practice standards.
This position will also play a key role in patient and family engagement by participating in care conferences, addressing concerns with empathy and professionalism, and promoting shared decision-making that respects patient values and cultural backgrounds.
