Clinic LPN

Rochester Regional Health is seeking a compassionate and detail-oriented Clinic LPN to join our Healthcare & Medical Services team.

In this role, you will provide direct patient care in an outpatient clinic setting, supporting physicians, advanced practice providers, and the broader care team to deliver safe, high-quality, and efficient services.The Clinic LPN will room patients, obtain and document vital signs and medical histories, perform medication reconciliation, and prepare patients for examinations and procedures.

You will administer medications and immunizations under provider orders, perform basic clinical procedures such as wound care, point-of-care testing, and specimen collection, and ensure accurate, timely documentation within the electronic health record.

You will assist with care coordination by following up on test results, reinforcing provider instructions, providing patient education on treatments and chronic disease management, and helping schedule follow-up visits and referrals.Working closely with nurses, medical assistants, and providers, the Clinic LPN will contribute to a collaborative and supportive environment that reflects Rochester Regional Health’s commitment to diversity, inclusion, and patient-centered care.
