HarmonyCares is seeking a compassionate CHA to join our Healthcare & Medical Services team, providing high-quality home-based care to patients and families in their own homes.

As a key member of our multidisciplinary care team, you will deliver direct clinical services, coordinate individualized care plans, and offer education and emotional support that empowers patients to manage their health with dignity and independence.

In this role, you will conduct comprehensive in-home assessments, monitor vital signs and symptoms, administer medications and treatments as ordered, and provide skilled interventions such as wound care, catheter care, and chronic disease management.

You will collaborate closely with physicians, therapists, social workers, and caregivers to develop and update patient-centered care plans that align with each patient’s goals, preferences, and cultural values.

Using sound clinical judgment, you will recognize changes in condition, communicate promptly with providers, and adjust care interventions to promote comfort, safety, and optimal outcomes.

Documentation will be completed accurately and timely in electronic medical records, ensuring regulatory compliance and continuity of care.

Because HarmonyCares specializes in home health, palliative care, and hospice services, you will often support patients with complex or life-limiting illnesses, offering education on symptom management, medication adherence, and home safety, while guiding families through difficult decisions with empathy and respect.

You will work with a high degree of autonomy in the field, organizing your daily visits, prioritizing urgent needs, and maintaining clear communication with the office team to coordinate schedules and services.
