LPN Evenings Strong Skills, Steady Shift

SOUTHERN HEALTH PARTNERS INC is seeking a compassionate and reliable LPN for an evening shift role, LPN Evenings (Strong Skills, Steady Shift), within our Healthcare & Medical Services team supporting correctional facilities.

In this position, you will provide direct, hands-on nursing care to an incarcerated patient population, ensuring safe, respectful, and compliant medical services in collaboration with RNs, physicians, mental health providers, and facility staff.

Typical duties include performing focused assessments and documenting findings, administering medications (including oral, IM, and other ordered routes), completing treatments and wound care, collecting specimens, and monitoring patients with chronic and acute conditions.

You will respond to sick call requests, triage and prioritize care needs, assist with intake screenings, and support chronic care clinics and medication passes, all while maintaining strict adherence to security procedures and state and facility regulations.

The role requires accurate and timely charting in electronic medical records, following standing orders and clinical protocols, and escalating changes in patient status to the appropriate provider.

You will participate in emergency response within the facility, follow infection control and safety policies, and contribute to a clean, organized, and secure clinical environment.
