New Graduate Nurse Practitioner

Oak Street Health seeks a New Graduate Nurse Practitioner to join our patient-centered primary care team serving adults on Medicare in underserved communities.

In this role, you’ll provide comprehensive assessments, chronic disease management, preventative care, and same-day visits in a collaborative, multidisciplinary environment.
