Job Description & Requirements Specialty: Palliative Care Discipline: Nurse Practitioner Duration: Ongoing Employment Type: Staff Work Schedule: 100% FTE, day shift working Monday - Friday, 8a - 5p. Be part of something remarkable As an Advanced Practice Provider, you'll play an integral role on our team by providing high-quality, cost-effective care to patients. We are seeking a Nurse Practitioner or Physician Assistant to: Provide complex symptom management recommendations. Work in collaboration with the Palliative Care Medical Director, attending medical team, and Palliative Care Team members to deliver palliative care to patients in the acute care setting with life-limiting chronic diseases. Effectively communicate with patients and family by utilizing concepts of motivational interviewing. Perform assessment process to include patient history, physical, psychosocial, and spiritual assessment. Provide support, counseling, and information to patients and families dealing with end-of-life issues or with issues related to life-threatening or serious and complex illness. Education: Minimum – If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program. If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program. Licenses and Certifications: Minimum - If Nurse Practitioner: Active RN and APNP license in State of Wisconsin AND active board certification as a NP in Family, Pediatric Acute Care, or Pediatric Primary Care. If Physician Assistant: Active PA license in State of Wisconsin AND active PA board certification.
