City: Toms River State: NJ Start Date: 2025-07-21 End Date: 2025-10-20 Duration: 13 Weeks Shift: 9 Hours Day shift Description: Start Date: July 21st or Eariler Duration: 90-day assignment confirmed by client Location: Home visits across Ocean County 6 providers needed. Please specify if your provider is comfortable starting his/her day in Ocean County. Scheduling: Must be available between 8 AM – 8 PM Scheduled for an 8-hour shift within this window e.g. 8 AM–5 PM or 12 PM–8 PM One-hour lunch break included Schedules are released 2–3 weeks in advance Full-time 40 hours/week Monday through Saturday flexible within that window Job Details: Conduct 5–6 home health assessments per day Paid for a full day regardless of patient volume Perform annual risk assessments and review of chronic conditions No treatment or prescribing – assessment-only role Escalate urgent findings to the patient’s PCP If applicable conduct A1C tests only other screenings left for the patient Travel required mileage reimbursed daily Equipment: iPad Blood pressure cuff Lab kits for patient use Credentialing: 1–2 week turnaround Must have an active NJ license and NJ Medicare number Qualifications: Must be Board-Certified FNP or ACNP Must be licensed in NJ Mileage will be reimbursed daily Will not cover travel Shift Info: Day 5x8-Hour 08:00 - 17:00 Notes: N/A 1099 Pay Rate: $111.15 Certification Requirements: Please confirm credential requirements with VHS upon application.
