Job Description & Requirements Specialty: Care Manager Discipline: RN Start Date: 07/28/2025 Duration: 51 weeks 40 hours per week Shift: 8 hours, days, nights, flexible Employment Type: Local Contract Job Title: RN Case Manager – Home Health Specialty Location: Phoenix, AZ Schedule: Full-Time Pay Range: $43.27 – $62.50 per hour (based on experience) Job Summary: We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our Home Health Specialty team in Phoenix, AZ . As a case manager, you will play a vital role in overseeing patient care, ensuring high-quality outcomes, and supporting patients throughout their home health journey. This is a dynamic position that combines clinical expertise with care coordination and patient advocacy. You will manage a caseload of patients in their homes, providing direct nursing care, conducting assessments, developing care plans, and collaborating with physicians, therapists, and other team members to ensure optimal health outcomes. Key Responsibilities: Manage a caseload of home health patients with complex clinical needs Conduct in-home patient assessments and develop individualized care plans Provide direct nursing care, including wound care, medication administration, and symptom management Coordinate multidisciplinary care and communicate with physicians and specialists Educate patients and families on condition management and treatment plans Monitor patient progress and update care plans as needed Maintain accurate, timely clinical documentation in compliance with regulatory standards Requirements: Active RN license in the state of Arizona (required) Minimum 1 year of nursing experience (home health or case management preferred) Strong clinical assessment and critical thinking skills Knowledge of Medicare guidelines, OASIS documentation (preferred) Exceptional communication, time management, and organizational skills Valid driver’s license, reliable transportation, and proof of auto insurance CPR certification (required) Benefits: Competitive hourly pay: $43.27 – $62.50/hour Flexible scheduling and manageable caseloads Mileage reimbursement Supportive and collaborative clinical team Paid time off (PTO) and holiday pay Medical, dental, and vision insurance options Ongoing training and professional development Retirement plan options About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
