Additional requirements include: Current licensure applicable to professional affiliation required. Certification by the National Association of Healthcare Professionals or InterQual is desirable. Certification as CCM or ACM desired. Department/Unit Summary Join our dedicated healthcare team where you'll assist with discharge planning, ensuring patients receive the necessary resources for a smooth transition from the hospital. You'll coordinate with third-party payers, government programs, and community services to arrange appropriate care, housing, transportation, and other essential services. Your role will involve coordinating placement and support services, including long-term care, rehabilitation, behavioral/mental health, home care, transportation, and DME, as well as assisting with CPS, APS, adoptions, and end-of-life planning, including advance directives. Additionally, you'll assist with advanced directives, end-of-life planning, and work closely with social services. This position offers an opportunity to make a direct impact on patient care by helping navigate complex discharge processes and fostering continuity of care.
