✨ Step into a leadership role focused on high-quality care for frail and elderly populations Full-time, Monday–Friday schedule with comprehensive clinical oversight Serve in a PACE (Program of All-Inclusive Care for the Elderly) environment—helping seniors remain independent and thriving in the community Collaborate with interdisciplinary teams of compassionate providers Focus on clinical outcomes, quality improvement , and mentoring APPs and PCPs Responsibilities : Conduct physicals, assessments, treatment planning, and periodic evaluations Direct and mentor a clinical team in delivering geriatric-centered care Lead Quality Improvement initiatives and implement care protocols Participate in on-call coverage and interdisciplinary meetings Collaborate closely with caregivers, families, and other professionals Educate participants and staff on health conditions and wellness
