Physician (MD/DO) - Internal Medicine - General/Other in Indiana

We are seeking a Board-Certified Family Medicine or Internal Medicine physician with Fellowship training or Certificate of Added Qualifications in Geriatric Medicine to serve as Medical Director for our Program of All-Inclusive Care for the Elderly (PACE). In this leadership role, you’ll oversee participant care, guide a multidisciplinary team, and help seniors with chronic care needs live independently for as long as possible. Full-time, day shift schedule – No weekends / No call Direct oversight of participant care, clinical outcomes, and QAPI program Lead and support Primary Care Providers (PCPs) and APPs Collaborate with hospital leadership, nursing staff, and community providers Participate in care planning, family conferences, and interdisciplinary meetings Conduct exams, develop treatment plans, and manage chronic conditions for frail older adults Serve as a key voice in shaping medical policy, protocols, and quality initiatives
