You will work with patients recovering from injuries or illness, helping them move better, feel better, and stay active under the guidance of a licensed Physical Therapist.

Minimum Qualifications Associate degree in Physical Therapist Assistant from an accredited educational program.
Active NC State PTA license required prior to start.
BLS certification required (AHA or ARC accepted).
Minimum of 1 year of Home Health experience required.
Homecare Homebase (HCHB) experience required (no exceptions).
Valid driver’s license and proof of vehicle insurance required.
Required skills
gait training, balance training, non-pharmaceutical pain management, effective communication with PT and clinical team, and ability to recognize signs and symptoms to help prevent readmissions.
Preferred
Experience with orthopedic postoperative care (total hips/knees), chronic disease management, and care of patients with CHF or COPD.

Average caseload
approximately 6 visits per day.
Typical driving
average 60–80 miles per day; mileage reimbursed weekly.
Technology/equipment
EPIC Rover (view only), Focura for document uploads, Samsung Note 9 phones, Homecare Homebase (HCHB) required.
Orientation
new employee orientation plus a 5-day clinical orientation.
Schedule notes
no weekend rotation, no on-call, no major holiday coverage; schedule cycles every 13 weeks; schedule changes require manager approval.
Floating
position will mostly cover northern Charlotte suburbs including Huntersville, Cornelius, Davidson and nearby communities; candidate should be prepared for geographic coverage within the region.
