Pay

$34.13/hr, bi-weekly Schedule

Monday–Friday, 8:00 AM–5:00 PM (40 hrs/week) Work setup

~75% local travel to member homes, assisted living facilities, and nursing homes.

Location (REQUIRED)

Applicants must currently reside in West Miami or Doral (Miami-Dade County ) Required Qualifications Fluent bilingual Spanish/English (read, write, and speak) — required Bachelor’s degree required (Social Work or related field) Verifiable High School Diploma or GED — required Case management experience — required Long-term care experience What You’ll Do Coordinate case management activities for Medicaid LTC/Comprehensive Program members Complete comprehensive member assessments using care management tools and data review Develop, coordinate, and monitor care plans; track progress and outcomes Collaborate with providers and multidisciplinary teams to support optimal care Identify/escalate quality-of-care concerns through established channels Assist with services/supports such as prior authorizations, PCP coordination, medication review, condition management education, and community resources Use motivational interviewing/influencing skills to drive engagement and healthy behavior change Document and follow case management processes aligned with regulatory/accreditation guidelines
