• Shifts include: Monday - Friday 8:00a - 5:00p, Call is one week per month, Weekday call 5:00p - 8:00a, Weekend Call 8:00a - 8:00a
• Duties entail: annual assessments, Post-hospital visits, Acute visits
• MA license in hand - Required
