• Full-Time/ Permanent Role
• Mon-Fri; 8 hour days
• Flexible scheduling
• Must be willing to commute with a 20 mile radius
• Provide community-based medical and care coordination services for recent hospital discharges. Manage a small panel of homebound members who require primary care. Resolve acute issues in the home and clinic or by directing members to the most effective and efficient solutions
• OH license in hand
• 2 years of direct experience is required
