Practice Setting: Hospital Requirements: Board Certification: Board Certified License required: AZ License DEA: Yes CSR: No Minimum experience required: 2 years Credentialing Timeframe: 30 days Shift Info: Night 3x12-Hour 18:00 - 06:00 Notes: N/A 1099 Pay Rate: $129.68 Certification Requirements: Please confirm credential requirements with VHS upon application.
